May is Older Americans Month

The WWHD is pleased to celebrate and acknowledge May’s Older Americans Month. The Administration for Community Living selected this year’s theme “Make Your Mark” to encourage and celebrate the countless contributions that older adults make to our communities. Their time, experience, and talents benefit family, peers, and neighbors every day. This year’s theme highlights the difference everyone can make – in the lives of older adults, in support of caregivers, and to strengthen communities.

The Health District reminds residents that there is a wealth of resources for seniors in our community. Our Home Bound Health Check program is one such example. This unique program provides regular non-skilled home visits by a registered nurse to residents unable to leave their homes due to chronic illness or advanced age. The goal is to help older or chronically ill residents of any age to stay at home and remain independent for as long as possible.

Services are tailored to meet individual health needs and include monitoring of vital signs, blood sugar levels and medication compliance. The nurses will also provide routine vaccines like flu shots and assess the home for safety hazards to prevent falls. Referrals and resources will be made as appropriate.

Who is eligible for Homebound Health Check? Westport and Weston residents of any age who are unable to drive and obtain needed services outside the home without experiencing undue physical or emotional hardship. Clients must not be receiving home health services from a visiting nurse agency. There is no fee to eligible participants. Although in-home visits are suspended due to the COVID-19 crisis, our nursing staff continues to check in with residents via telephone. Enrollment is open.
